One day a consultant in a Banjara Hills hospital said that the second dome was the best money the management managed to spend. A surgeon at a smaller facility at LB Nagar stated this week that his second dome is pushed out of the way most of the time.
Both were right. The difference was the case mix.
There are not many decisions in OT lighting where you can compare specs to determine what is best but a single dome, double dome and satellite arrangement can be one of those few. This will rely completely on what occurs in your theatre.
When a single dome is enough
A single dome is adequate for a day-care theatre, a minor operating theatre, a dressing room or a nursing home performing routine general surgery and short cases.
Lighter on ceiling, lower cost, easy to clean around and quick to reposition. If you have a lot of procedures that are less than one hour, and have shallow surgical fields, then a good single dome with good depth of illumination is going to outperform any mediocre double dome.
What has been done wrong is the purchase of a poor quality single dome followed by the purchase of a second dome two years later to make up for it. This is more expensive than obtaining one good unit up front.
When you genuinely need two
A second dome is necessary, and it is important when using three or four people leaning over the same field, for shadow control.
Single dome is getting blocked when doing deep cavity surgery, obstetrics, ortho, long laparoscopic surgery with an assistant and scrub nurse over the site. The second dome allows you to illuminate the field from another angle so that a hand or a head in the way doesn’t affect the illumination of the cavity.
A double dome provides a back-up, as well. If one head fails during the procedure, the case proceeds. That’s a redundancy in and of itself for a hospital that maintains emergency lists.
The satellite option
A smaller secondary dome paired with a large main dome is common in theatres that are tight on ceiling space or budget.
It is a reasonable middle path. Just be clear about the satellite specification — some are genuinely useful working lights, and some are little more than an examination lamp bolted to an arm. Ask for the lux and CRI of the satellite head separately. If the supplier only quotes numbers for the main dome, that tells you something.
Ceiling space is the constraint nobody plans for
Here is where theatre design and equipment selection collide.
A double dome needs a central mounting point and enough clearance for both arms to swing without hitting each other, the pendant, or the laminar flow canopy. In older theatres, especially conversions, that clearance often is not there.
Measure the slab height, the false ceiling gap and the existing pendant positions before finalising the configuration. Any supplier of OT light manufacturers in Hyderabad worth dealing with will do a site survey rather than quote off a phone description. If they are willing to skip that step, they will also be willing to skip others.
Do not forget the mobile light
At the very minimum, every hospital should have one of these.
Power cuts happen. In emergencies, they are transferred to rooms which were never intended as theatres. A trolley mounted LED surgical light, with battery backup, is much cheaper than a ceiling mount, and has saved a lot of procedures in this city. Buy one. Keep it charged.
What to confirm regardless of configuration
No matter which arrangement you select, the following questions come to mind.
Colour rendering index > 95. Adjustable colour temperature. Low heat at the surgical field—a thing not to be taken lightly during a Hyderabad summer, as many brochures would suggest. A spring arm that does not drift. Sterilisable handles provided, extras can be bought. Also a written warranty that detaches the arm from the dome.
After that, the application ones. What is the closest Service Engineer? Do the driver boards and LED modules have to be stocked in India? How many spares will be on hand for this particular model?
An established OT light manufacturer in India with its own production line can answer all of that in one call. An importer usually cannot.

Match to your top 5 most frequent procedures. Purchasing a strong dome rather than two weak ones. Make sure to measure the ceiling first before you make a decision. And reserve a mobile unit.
